In its current public mode, this site does not accept online requests, use analytics, display advertising, or store personal information in your browser. Phone and email links open the applications configured on your device.
A production hosting provider has not been selected. Before launch, this policy must be updated to accurately describe technical request data and logs processed by that provider, without inventing retention periods.
Google Map
Google Maps content loads only after you select “View map.” When you do, Google may receive technical data, including your IP address, under its own policies.
Email and calls
Standard email and phone calls are not part of a secure patient portal. Do not send medical or confidential information by standard email. This website policy does not replace a clinical notice of privacy practices.
Pending review
Before launch, the agency must approve this policy, the final domain, providers, and any future form or analytics flow.
